Instructions:

All proposals must be submitted by an undergraduate/graduate student, faculty, staff, or campus department of UC Berkeley. If not primarily submitted by a student, all proposals must have at least one student sign as a co-sponsor of the project.

Proposals will be reviewed by the Wellness Fund Advisory Committee. Anyone listed on this form may be contacted for additional information.

Proposals should align with the goals of the original Wellness Referendum, as stated on: http://bit.ly/wellnessreferendumfee.

Funding Information








If yes, please fill out the Supporting Materials document linked below with the information for your partial budget, include the essential components of your program and their costs.
Budget

Please use this Supporting Materials template for the following:

  • Proposed Budget
  • Budget for Partial Funding
  • All non-Wellness Fund sources you are pursuing for funding, volunteer time, in-kind donations, etc., indicating the source of donation, amount of donation, date of when the donation request was submitted and when the donation was received.
